Are you new to Mohican Sports Medicine? The
following information is provided to guide you through your first
visit and your care at one of our outpatient clinics. We look forward
to serving you.
Before Your First Visit
- Prescription for Treatment: Most
patients come to us through a physical therapy referral from their
physician. Physicians typically include a prescription/recommendation
for treatment with a referral.
- Appointment Scheduling: Before
your first visit, you will need to schedule
your initial evaluation at one of our outpatient
clinics.
- Insurance Verification:
Our friendly and courteous staff will work with you to verify
your insurance and our participation in your specific insurance
plan.
At Your Initial Evaluation
- What to Wear: In most situations,
we recommend that you wear comfortable, loose-fitting clothing
and sneakers appropriate for exercise.
- Registration Forms: Please arrive
15 minutes prior to your scheduled appointment time to allow time
to complete patient registration forms. When you arrive at the
clinic, our receptionist will greet you and help complete the
necessary paperwork. Following your initial visit, the receptionist
will schedule any needed follow-up appointments and collect your
co-payment (if applicable).
- Meet Your Therapist: You will
meet with a Physical Therapist or Occupational Therapist who will
evaluate your injury or condition, discuss your goals and any
specific needs, and then develop your personal treatment plan.
- Your Treatment Plan: The length
and frequency of your treatment plan is determined by your specific
injury or condition, the physician’s prescription/recommendations,
your therapist’s recommendation and your insurance plan.
Regularly Scheduled Appointments
Most likely, you will have a few therapy appointments
each week. While these appointments may present a temporary inconvenience,
these regular visits are critical to your long-term progress. At
each visit, you will work with one of our specialists who will instruct
and manage your care.
As you progress, new levels of treatments and exercises
will be introduced to help you reach specific treatment goals. Following
each of your visits, your therapist will meet with you to discuss
your progress and answer any questions you might have. During your
care with us, your therapist will also communicate regularly with
your physician, surgeon and/or case manager to provide timely updates
on your progress.
Home Exercise Programs
Our therapist often prescribe home exercise
programs to supplement or complement your plan of care. These programs
are an important part of recovery and will assist you in achieving
your goals.
